NY State Health Care Proxy Form

Maintained by the NYS Department of Health, this website explains what a health care proxy is and includes a downloadable health care proxy form with instructions.

NY State Office of Long Term Care Ombudsman

This program investigates and resolves complaints and concerns about skilled nursing facilities and long term care facilities such as adult homes. Volunteers are recruited and trained as certified ombudsmen who advocate for residents, assisting them, their families and the facilities in resolving problems.

NYS Attorney General’s Office

Consumer frauds are most often targeted at elders. About 85 percent of the victims of telemarketing fraud are age 65+. Scams include, but are not limited to: home repair; foreign lottery or sweepstakes solicitations; investments; and charities fraud. Elders targeted by fraud should contact the Attorney General’s office.
